-----------------------------------------------------------------

Much of the work happens before and after we meet, making our short time together the most productive.

Using the prompting questions (below), which you can answer, adding your own questions and concerns. You can then send all of these to me, up to three days before our appointment, and I will come to our meeting with resources, tools, and stratgies tailored to your situation. Together we will make an action plan, and afterward I will send follow up materials and resources depending on your need.

We may want to schedule a follow up, but it is entirely up to you. For ongoing support, we can talk about my other services: check-in calls, brief consults, and counseling.

PROMPTING QUESTIONS

·        Do you have family or a close friend that you may want to involve in this conversation? What are your goals for you/your loved one? Your family’s goals for your/their care?

·        What supports are in place already?  (examples: respite care, breaks for you to get away, family members who help out, etc.)

·        What supports are most needed at this time?   What supports do you anticipate you/your loved one needing in the future?
